To initiate Intensive In-Community (IIC) services for individuals aged 0 to 21 years, please contact PerformCare by calling 1-877-652-7624. During the call, request Modra Health to serve as your designated service provider.
Virtual Support Services
To access telehealth services that address mental and emotional health, or stress-related concerns, please complete the virtual service request form available on our website. A licensed mental health professional will contact you to initiate an individualized assessment and discuss appropriate next steps for support.
